Komplexní molekulární diagnostika gastrointestinálních stromálních nádorů ABSTRAKT Většina gastrointestinálních stromálních nádorů (GIST) je charakterizována přítomností aktivačních mutací v genech KIT a PDGFRA (80 - 85 %). Ve skupině KIT/PDGFRA nemutovaných GIST (10 - 15 %) byly detekovány mutace v genech BRAF, KRAS, defekty v komplexu SDH nebo fúze FGFR1::TACC1, ETV6::NTRK3. Cílená terapie Imatinib mesylátem (IM) znamenala významný průlom v terapii GIST. Výrazným problémem cílené terapie je vznik primární nebo sekundární rezistence. Biologické chování GIST je nepředvídatelné. Stanovení proliferační aktivity vybraných markerů proliferace a senescence umožní objektivizovat riziko agresivního chování nádoru. Cílem první části naší práce byla mutační analýza genů KIT a PDGFRA a detekce sekundárních mutací u pacientů s progresí onemocnění. V 83.5 % vzorků jsme detekovali primární mutace v genech KIT a PDGFRA. U 16 pacientů s progresí onemocnění jsme prokázali přítomnost sekundárních mutací (v 29 progresivních ložiscích - 69 %). Potvrdili jsme výraznou intranádorovou a rovněž internádorovou heterogenitu sekundárních mutací. Cílem druhé části práce byla analýza alterací ve skupině KIT/PDGFRA nemutovaných GIST. Complex molecular diagnostics of gastrointestinal stromal tumors ABSTRACT Most of gastrointestinal stromal tumors (GISTs) are characterized by activating mutations in the KIT and PDGFRA genes (80 - 85 %). Mutations in the BRAF, KRAS, defects in the SDH complex or FGFR1::TACC1, ETV6::NTRK3 fusions were detected in the group of KIT/PDGFRA non-mutated GISTs (10 - 15 %). Targeted therapy with Imatinib mesylate (IM) marked a significant breakthrough in treating GISTs. A significant problem with targeted therapy is the formation of primary or secondary resistance. The biological behavior of the GIST is unpredictable. Determining the activity of selected markers for cell proliferation and senescence will enable the objectification risk of tumor behavior's aggressivness. The aim of the first part of our work was the mutational analysis of the KIT and PDGFRA genes and the detection of secondary mutations in patients with disease progression. We detected primary mutations in KIT and PDGFRA genes in 83.5 % of samples.
