Ošetřovatelská péče u pacienta s torakoabdominálním aneuryzmatem

Abstract

(ČJ) V mé bakalářské práci se zabývám problematikou torakoabdominální výdutě aorty (TAAA). Je to velmi závažné onemocnění sestupné aorty, které vzniká zeslabením, nebo poškozením cévní stěny a může ohrozit pacienta na životě. Chirurgická intervence je nutná už od dilatace nad 5 cm. Ruptura aneuryzmatu je život ohrožující stav, často neslučitelný se životem. Operace tohoto onemocnění se provádí pouze na vysoce specializovaných pracovištích. Hlavním cílem této práce je prezentovat onemocnění TAAA, jeho diagnostiku, léčbu a ošetřovatelskou péči od příjmu až po propuštění. Dílčím cílem je identifikovat faktory, které ovlivňují průběh rekonvalescence. Praktická část této práce je zpracována formou kvalitativního výzkumu, technikou případové studie pacientky, přijaté k operaci TAAA, a analýzou zdravotnické dokumentace v průběhu celé hospitalizace. Dalšími technikami šetření je pozorování a nestrukturovaný rozhovor se sestrami pečujícími o tuto pacientku. Vzhledem k tomu, že o ošetřovatelské péči u pacientů s TAAA je velmi málo literatury pro sestry, vytvořila jsem v závěru své práce edukační materiál pro sestry v adaptačním procesu, který bude k dispozici pro Oddělení cévní chirurgie Nemocnice Na Homolce (NNH). (AJ) The theme of my Bachelor's thesis is a Thoracoabdominal aortic aneurysm (TAAA). It is a very serious disease of the descending aorta, which arises weakening or damage of the vascular wall and can endanger the patient's life. Surgical intervention is necessary once the size of the dilation is 5 cm or more. Aneurysm rupture is a life-threatening condition, often leading to death. Surgical intervention of this disease can be done in highly specialized centres only. The primary target of this document is to present the TAAA disease, its diagnostics, cure and care from the patient's admission to his/her discharge. The secondary target is to identify the factors that affect the course of convalescence. The practical part of this thesis is composed in the form of a qualitative research, as a case study of a female patient admitted for the TAAA surgery, and by analysis medical records during the entire hospitalization. Another part of research is an observation and an unstructured interview with nurses taking care of the patient.
