Vyšetření hemostázy u dětí před otorinolaryngologickými operacemi

Abstract

Souhrn: Vyšetření hemostázy u dětí před otorinolaryngologickými operacemi Úvod: Krvácivé komplikace v souvislosti s adenoidektomií (AT) a tonzilektomií (TE) zůstávají významným medicínským problémem, navzdory vývoji operačních postupů a technologií. Záměrem předoperačního vyšetření hemostázy je identifikace pacientů s krvácivou chorobou a tedy vyšším rizikem krvácivých komplikací. V poslední době je diskutován nejefektivnější algoritmus vyšetření hemostázy před otorinolaryngologickými operacemi. Hlavními cíli této práce jsou: zjištění výskytu abnormalit koagulačních testů u dětí před AT a TE, odhalení příčin těchto abnormalit a zhodnocení vlivu koagulačního vyšetření na další léčebný postup. Soubor a metody: K naplnění těchto cílů byly zhodnoceny dva soubory pacientů. Prospektivně byl vyšetřen koagulační profil včetně standardizované anamnézy dle dotazníku u souboru pediatrických pacientů indikovaných k AT nebo TE - soubor č. I. Byl zjišťován výskyt abnormalit předoperačních koagulačních testů a jejich vztah k peri-/postoperačnímu krvácení. Dále byl retrospektivně analyzován soubor pacientů referovaných do hematologické poradny pro abnormální předoperační koagulační vyšetření - soubor č. II. Haemostasis testing in children before ear-nose-throat sugery Introduction: Haemorhagic complications in association with adenoidectomy and tonsillectomy are still a significant medical problem, despite development of surgical procedures and technology. The purpose of the preoperative coagulation tests is to identify patients with a bleeding disorder and therefore with a higher risk of bleeding. There is ongoing discussion about the most effective preoperative screening before ear-nose-throat surgery. The main aims of this study are: to find out the occurence of abnormal preoperative coagulation test results in paediatric patients before adenoidectomy and tonsillectomy, as well as to identify the most frequent causes of prolonged coagulation tests and to evaluate the influence of coagulation screening for further perioperative management. Patients and methods: Two cohorts of patients were evaluated. Cohort I.: In this prospective study coagulation tests were performed in children before adenoidectomy and tonsillectomy. A questionnaire was used for asking parents about the personal and family history of bleeding symptoms.
