Case management a jeho význam pro člověka se schizofrenií

Abstract

Tato diplomová práce popisuje kontext case managementu pro lidi se schizofrenií v České republice. Práce je rozdělena do tří kapitol. První kapitola je věnována systému péče o lidi s duševní poruchou, jejímu vývoji a jednotlivým kategoriím. Tématem další kapitoly je schizofrenie, tedy duševní porucha nejvíce se vyskytující mezi klienty využívající služby case managementu. Poslední a stěžejní část této práce je kapitola o case managementu, jeho vzniku, modelech, zásadách a uplatnění v péči o duševně nemocné klienty. Poslední kapitola také popisuje fáze práce s klientem, zabývá se osobou case manažera, postavením klienta a charakteristikou týmu case managementu. Na závěr jsou uvedeny dva rozhovory s case manažery.

This thesis is describing a context of case management for people with schizophrenia in the Czech republic. The thesis is divided into three chapters. The first chapter is dedicated to a system of the care of people with a mental disorder, its evolution and individual categories. The topic of the next section is schizophrenia, the most common mental illness among clients utilizing case management services. The last and crucial chapter of this dissertation deals with case management, its creation, models, principles and its aplication in the care of mentally ill patients. The last part also depicts phases of dealing with client, treats of the case manager, status of the client and the case management team characteristics. At the conclusion two dialogues with experts at case management are cited.
