Adiktologická péče o klienty s duální diagnózou v Centrech duševního zdraví pro osoby se závažným chronickým duševním onemocněním

Abstract

Background: According to the standards of the Mental Health Center services, their function is integrated care for persons with a dual diagnosis (MZČR, 2021). Objectives: The goal of the work is to describe and understand how and by whom care is provided to clients in the field of addiction in selected centers. Methods: The practical part of the work consists of semi-structured interviews with workers from 15 (52%) different centers involved in the care of clients with a dual diagnosis in the field of addiction, processed using the method of thematic analysis. Main results: Respondents from 87% of the examined centers had some theoretical or practical training in the field of addiction. Addictologists worked in 40% of the examined centers, all of them in the position of social worker. Those in the center fulfill the role of specialists in working with people with a dual diagnosis. Most centers combine different treatment models. If the client is indicated for residential addiction treatment, they apply the model of successive treatment. They widely use the parallel treatment model. If necessary, they integrate elements of addiction treatment into the care provided. Východiska: Dle standardů služeb Center duševního zdraví (CDZ) je jejich funkcí integrovaná péče o osoby duální diagnózou (MZČR, 2021). Cíle: Cílem práce je popsat a porozumět, jak a kým je ve vybraných CDZ poskytována klientům adiktologická péče. Metody: V praktické části práce jsou metodou tematické analýzy zpracovány polostrukturované rozhovory s pracovníky z 15 (52 %) různých CDZ, kteří se podílejí na péči o klienty s duální diagnózou v adiktologické oblasti. Hlavní výsledky: Respondenti z 87 % zkoumaných CDZ měli nějakou teoretickou či praktickou průpravu v adiktologické oblasti. Ve 40 % zkoumaných CDZ pracovali adiktologové, všichni na pozici sociálního pracovníka. Ti v CDZ plní roli specialistů na práci s lidmi s duální diagnózou. Většina CDZ kombinuje různé modely léčby. Model sukcesivní léčby aplikují, když je klient indikován na pobytovou adiktologickou léčbu. Hojně používají model paralelní léčby. Je-li to nutné, integrují do poskytované péče prvky adiktologické léčby. Pouze v jednom zkoumaném CDZ probíhají skupiny pro klienty s duální diagnózou. Závěr a doporučení: Různorodá průprava členů týmů CDZ v adiktologické oblasti se jeví jako nesystematická a často nedostatečná.
